I confirm I will complete this form myself and answer the questions to the best of my knowledge.

I confirm I am legally entitled to live and work in the UK. 

I must have held a full, clean UK driving licence (excluding minor traffic offences) for a period of at least 24 months at the time of applying. Any licence I hold from an EU member state must have been held for at least 12 months. I understand any points on my driving licence(s) will be considered when applying.

I will disclose any previous convictions. I understand that I may be referred to the Licensing Committee for a decision on my application if my disclosure certificate shows relevant convictions.

If I am a foreign national and arrived in the UK within the last 12 months, I will need to bring the equivalent disclosure notice from the country I was resident in for the last five years to my interview.

When I receive my DBS (Disclosure and Barring Service) certificate, I'll bring it in for checking.

If my first language is not English, I will be required to take and pay for a comprehension test. This will be explained at the interview stage in person.

I understand I must pass a knowledge test, with at least an 80% pass mark on each paper. If I fail any module of the test, a resit can be taken at any time but it will be the complete test. I understand I must pay the resit cost, which is still payable even if I don't attend a booked test appointment.
