This form must be completed by the care provider of the tenant claiming housing benefit.
The answers you give us will be shared with the Department for Work and Pensions.
Information you will be asked for
- personal information about the tenant, including their date of birth, National Insurance number and their address
- contact details for their social worker or support worker if they have one
- details of the care and support they are given, including a care plan if they have one
- the type of property they live in
To pay housing benefit we need:
- their full tenancy agreement
- a full breakdown of their rent and charges
- a completed housing benefit claim form
- a signed copy of a care plan (or similar) for the tenant - you can take out any personal information
